Description
We are currently seeking a Personal Care Support Worker for an individual with special needs in our home on weekend and evenings.
In this role, you will work closely with the individuals to ensure their physical, emotional, and social needs are met. Your tasks will include assisting with personal care, such as bathing and dressing, toileting, medication, tube feeding, administering medication, and assisting with light household chores. Additionally, you will also be responsible for documenting client progress and communicating with the care team to ensure the best care possible. You will also be required to follow the routines that the physical therapist has given including using a walker, standing frame, and wheelchairs. The two individuals use AAC tactile cues for communication.
Responsibilities
· Provide personal care assistance to individuals in need, including bathing, dressing, toileting, and all other hygiene needs
· Offer companionship and emotional support to clients by engaging in meaningful communication using total communication and activities
· Administer medication according to prescribed schedules and document accurately
· Follow G and J tube feeding protocols
· Assist with light household chores, such as meal preparation, laundry, and cleaning
· Monitor and report any changes in client behavior or health conditions to the care team
· Document client progress and maintain accurate records
· Follow protocols outlined by the team for physical therapy
· Communicate effectively with clients, their families, and the care team to ensure coordinated care
Requirements
· High school diploma or equivalent
· Minimum 6 months of relevant work experience, previous experience with severe disabilities is preferred
· Strong interpersonal skills and the ability to build trust and rapport with clients
· Good communication skills, both verbal and written
· Ability to effectively manage time and prioritize tasks and work unsupervised
· Knowledge of basic healthcare procedures and safety protocols
· Compassionate, patient, and empathetic demeanor
· Valid driver's license or reliable transportation to the family home
· Criminal record check for the vulnerable sector that is clear
· Be available for 30 to 40 hours a week on evenings and weekends
